Since notice and HIPPA medical releases / disclosures are so important and must be so specific, why not give an example of an actual release that was used in the one of your cases that you litigated? You spoke for 45 minutes about notice without an example of a HIPPA form. I have looked online, Lexis Nexis, case law, etc.without success in finding an example compliant with 29-26-121. I know each case is different, however, it would be beneficial to have an example that was used successfully to defeat a motion to dismiss in an actual case. Respectfully submitted, Thank you.
